As to exfoliating, it's tricky! I still tend to very briefly use an apricot scrub on a day when I know I will be at home that day and the next, since it does cause a bit of redness and irritation. I also have found that using a green papaya enzyme powder, mixed with water and left on for no longer than 5 minutes, to work very well on the little flakies.
Sometimes I do get a little bit of irritation, depending on my skin at the moment, but for me, it's worth it. I find that my skin is softer, and better able to absorb my moisturizers, topical medications, etc.
Another option would be the Clarisonic or a similar brush ; I use it sometimes, with the "delicate" brush-head. I tend to push too hard though, and the key is to let the brush do the work--lightly skimming the surface of your skin rather than scrubbing it.
When I was diagnosed I was told not to exfoliate. I was given prescriptions and recommendations, and those worked well. However, I still felt the burden of thickened dry flaky areas. I went against what I was told. I found a product made by LUSH called "angels on bare skin" which is very gentle. It works very well for dry skin, does not strip skin oils, etc.
